2. Which Vitamin Code Multivitamin, Garden of Life Probiotics and
Minami Nutrition Omega-3 Supplement skus are included in the Big 3
promotion?
All of them:
Vitamin Code (all sizes): Women, 50 & Wiser Women, Men, 50 &
Wiser Men, Family, Perfect Weight, Liquid, Kids, RAW Prenatal, RAW One

3. Do consumers have to purchase at least one Vitamin Code
Multivitamin, one Garden of Life® Probiotics and one Minami Nutrition
Omega-3 Supplement in one transaction to be eligible for their $25
rebate check?
At least one sku of each of the three product groups (Vitamin Code
Multivitamins, Garden of Life Probiotics and Minami Nutrition Omega-3
Supplements) must be purchased in one transaction.
4. What do consumers need to submit with their rebate form to
receive their $25 rebate check?
Consumers must mail the original Big 3 Promotion Rebate Form
completely and legibly filled out and signed, a copy of their receipt or
invoice showing all three products purchased in a single transaction
(any one Vitamin Code Multivitamin, any one Garden of Life Probiotics
and any one Minami Nutrition Omega-3 Supplement) plus at least 2 entire
original UPC codes, one from each of the products packaged in cartons
(no photocopies or reproductions of the UPC codes will be accepted).
5. If a consumer does not include at least two of the entire
original product UPC codes with their rebate submission will they still
receive their rebate check? Will photocopies of the UPC codes be
accepted?
No. If a consumer does not include the entire original product UPC
codes from at least two purchased products, one from each of the
products packaged in cartons, with their rebate submission, they will
not receive their rebate check. No photocopies or reproductions of the
UPC codes will be accepted.
6. If a consumer does not include a copy of their receipt or
invoice with their rebate submission will they still receive their
rebate check? Will a photocopy of the receipt or invoice be accepted?
No. If a consumer does not include a copy of their receipt or invoice
with their rebate submission showing all three products purchased in a
single transaction (any one Vitamin Code Multivitamin, any one Garden of
Life Probiotics and any one Minami Nutrition Omega-3 Supplement) with
their rebate submission, they will not receive their rebate check.
Photocopies or reproductions of the receipt or invoice will be accepted.
7. Can consumers who use Garden of Life coupons to purchase Big 3
Promotion products also submit for the $25 rebate?
No. The Big 3 Daily Online $25 Rebate Promotion cannot be combined
with any other Garden of LIfe® coupon or offer. If the receipt or
invoice indicates a coupon was utilized to purchase any of the three
products (any one Vitamin Code Multivitamin, any one Garden of Life
Probiotics or any one Minami Nutrition Omega-3 Supplement), their rebate
submission will be declined.
8. Can consumers who receive an online retailer discount to
purchase Big 3 Promotion products also submit for the $25 rebate?
If the receipt or invoice indicates an online retailer discount was
applied (not a Garden of Life coupon), the rebate submission will be
accepted.
If the receipt or invoice indicates a coupon was applied, the rebate
submission will be declined.
If a consumer utilizes a coupon to purchase another product (not one
of the Big 3 Promotion products) and the receipt or invoice does not
clearly indicate what product the coupon was used for, the rebate
submission will be rejected. In this case it is recommended that a
separate transaction be utilized to purchase the other product with the
coupon.
9. Are there consumers who are not eligible for the Big 3 Daily
Online Promotion?
The Big 3 Rebate Promotion is limited to USA and Canadian addresses.
The following individuals and their families are not eligible for
participation: employees of Garden of Life LLC, its subsidiaries and
affiliates, printers, advertising and promotion agencies, manufacturers,
suppliers, importers, wholesalers and/or retailers.
10. How many times can a consumer participate in the Big 3 Daily
Online Promotion?
Consumers may participate as many times as they would like as long as
they submit the required documentation.
11. How many consumers per household address can participate in
the Big 3 Daily Online Promotion?
There is no limit to the number of consumers in a household who can
participate as long as each submits the required documentation.
12. How long will it take for consumers to receive their $25
rebate check?
Allow eight (8) weeks for the rebate submission to be processed and
the check received.
13. Is there a deadline for consumers to cash their $25 rebate
check?
The $25 Big 3 Promotion rebate check must be cashed within 90 days of
the issue date.
14. How can consumers check the status of their Big 3 Promotion
rebate submission?
Consumers can call 1-888-426-4650 toll free or visit
www.rebateinquiryonline.com for answers to questions or information
on their rebate submissions.
15. If a rebate submission is declined how is it communicated?
A letter will be sent communicating the rejection along with the
reason(s) for it. In the case of a submission rejection based upon the
receipt or invoice indicating use of a coupon to purchase product, a Big
3 Promotion Rebate form will be included with the letter so that the
consumer may participate and receive a rebate on their next purchase.
16. When does the Big 3 Promotion end?
Rebate submissions must be postmarked by December 31, 2012.
